WaTCHDOg stands for: Ward Round, Team, Communicate, Handover, Documentation, Outcomes. Liverpool Hospital ICU introduced a 2 page brochure for staff, to assist in the provision of a consistent approach to clinical handover, ward rounds, documentation, delivery of evidence-based care and a guide for complex/long stay patient review. Incorporated into the brochure is a simple outline of a Care Plan for the day.
