WaTCHDOg stands for: Ward Round, Team, Communicate, Handover, Documentation, Outcomes. Liverpool Hospital ICU introduced a 2 page brochure for staff, to assist in the provision of a consistent approach to clinical handover, ward rounds, documentation, delivery of evidence-based care and a guide for complex/long stay patient review. Incorporated into the brochure is a simple outline of a Care Plan for the day.

Community Care Zoning - Northern Sydney Central Coast Area Health Service

Care zoning provides a supervisory framework that supports a structured approach to clinical risk assessment and the ongoing monitoring.  These tools were developed in the context of acute mental health in-patient units and have been successfully used in a community setting.
