Safe Clinical Handover Program in NSW

"transfer of professional responsibility and accountability for some or all aspects of care for a patient, or group of patients, to another person or professional group on a temporary or permanent basis." 1,2,3
In 2008 the Special Commission of Inquiry into Acute Services in NSW Public Hospitals (Garling Report) recommended that each facility in NSW Health should have a mandated clinical handover policy within 18 months. This recommendation was supported in the Government response, Caring Together.
Improving clinical handover is also a high priority nationally, Australian Commission on Safety and Quality in Health Care, National Clinical Handover Initiative.
New ISBAR App
New South Wales Health and South Australia Health have collaborated to develop an ISBAR iPhone/iPad application, that is available free of charge on iTunes.
ISBAR- Introduction, Situation, Background, Assessment and Recommendation is a structured approach to clinical handover to improve patient safety.
The standard key principles for clinical handover provide a framework for implementation across a wide range of clinical handover scenarios, embracing the concept of ‘flexible standardisation’.
