Organisation of Care

Our patient and their carers are our experts to provide insight into how well our service provides coordinated care as they journey through our health system. They are well equipped to inform us where we need to improve our processes and acknowledge what we are doing well. Patients and carers appreciate good coordination of care and feel they have received an excellent heath journey when they experience organised care.

The solutions below showcase how some health services are working to improve in this area.

Solutions

This program aimed to provide comprehensive health assessments/screening of all primary school aged children in Dareton.

The Envelope is a big, bright yellow resealable envelope in which to put a resident's medical documents and other key information to 'go with' a resident of an aged care home when the resident is transferred to hospital.

This program aims to increase the uptake of falls prevention strategies for older people through phone follow ups within a week of discharge from the emergency department.

The Mums and Dads with Koori Bubs clinics have contributed to an increase in access for antenatal and postnatal care for Koori women and also for early childhood assessments.
