Procedure specific consent forms are available for patients, medical practitioners and other health care providers. The information in these forms includes the condition, the procedure itself, risks and benefits.

Be Informed: 12 Questions to Ask About Your Surgery

Are you facing surgery? Most operations are not emergencies. This means you have time to ask questions about the operation. To help you get the information you need, there are 12 questions you can ask your doctor about your surgery. These questions are presented in brochure, information sheet and video form on this web site. The video can also be taken out on loan from your local library or can be seen at the hospital.

Patient Information Sheets

Want more information about a procedure? These patient information sheets cover a variety of procedures undertaken in Queensland each year. The information presented includes the condition, the procedure itself, risks and benefits, other types of treatment available, and what you can expect after the surgery. There is also an information sheet about anaesthesia - which talks about different types of anaesthetic and the risks.

Multilingual Information

For those people who cannot read English, the "Be Informed" brochure, "Anaesthetic - Patient Information" information sheet and a generic consent form have been translated into eight different languages.
