
Project RED (Re-Engineered Discharge)
The Project RED (Re-Engineered Discharge) program run at the Boston University Medical Center focuses on:
- patient-centred education,
- discharge planning
- post-discharge reinforcement
The program recreates the discharge process to improve the process and post-discharge care.
Since the introduction of the Project RED there has been a reduction in post-discharge adverse events and a reduction of hospital ED visits and re-admissions by 30%. (Annals of Internal Medicine, 2009 as cited on Advisory.com 04/02/09).
