Coordination and Integration of Care

Patients report feeling vulnerable and powerless in the face of illness. Proper coordination of care can ease those feelings. Patients identified three areas in which care coordination can reduce feelings of vulnerability:

  • Coordination of clinical care.
  • Coordination of ancillary and support services.
  • Coordination of front-line patient care.

Below are some solutions to improve health services in this area.

  • Getting Medications Right - the First Time - North Coast Area Health Service
    Project to ensure the accuracy of charted medications for patients over 65 years on four or more medications admitted through the Emergency Department.

Organisation of Care

  • Aboriginal Child Health Check Program - Coomealla Health Aboriginal Corporation
    This program aimed to provide comprehensive health assessments/screening of all primary school aged children in Dareton.
  • Aged Care Home Transfer-to-Hospital Envelope - Chiminello, Clare
    The Envelope is a big, bright yellow resealable envelope in which to put a resident's medical documents and other key information to 'go with' a resident of an aged care home when the resident is transferred to hospital.
  • Follow up of Fallers - Hunter New England Area Health Service
    This program aims to increase the uptake of falls prevention strategies for older people through phone follow ups within a week of discharge from the emergency department.
  • Mums and Dads with Koori bubs clinic - Southern General Practice Network
    The Mums and Dads with Koori Bubs clinics have contributed to an increase in access for antenatal and postnatal care for Koori women and also for early childhood assessments.
