Ease of access to Healthcare professionals (other than Doctors and Nurses) while in our care has been highlighted as an essential factor influencing the experience of many of our patients and their carers. When patient and carers are able to interact well with Healthcare professionals they feel they have received excellent care and are better informed about their treatment and planned care.

The Northern Beaches (NBs) APAC/GP Shared Care model was initiated in 2006/2007 in response to increasing demand on Emergency Departments (EDs) and the growing number of elderly patients, with ambulatory sensitive conditions, being admitted to hospital.

Driving Change in Mental Health - Sydney West Area Health Service
The clinical redesign of community mental health in one of the sectors of SWAHS was identified as an area for improvement due to issues in service capacity. This was impacting on length of stay (LOS) of the inpatient unit as well as the number of clients seen by the service.
