Extended Day Only Model of Care

The Extended Day Only is a model of care that has improved certainty for patients, assists with reducing elective surgery delays, supports the management of waiting lists and reduces operating costs.  This model illustrates how the implementation of key elements of an Extended Day Only model can improve the patient journey and the delivery of surgical services.  The model is based on the premise that the majority of surgical care can be administered within a period of less than 24 hours. 

The adoption of key components such as a simplified booking and management system, appropriate patient selection, pre-admission assessment and preparation, flexible admission times, the use of clinical protocols and the insulation of beds for planned surgical services can dramatically improve the patient journey and staff satisfaction.

 

Lita has a painful knee. Her GP refers her to an orthopaedic specialist. She has an arthroscopy. Six months later her pain has increased and her mobility declined. The specialist recommends an arthroscopy at the EDO Unit that has a one month wait time. Lita will need support at home post surgery. Lita completes and returns her PPQ. The nurse screener contacts Lita to advise the surgery date. Her husband arranges annual leave to care for Lita post surgery. The hospital staff call Lita with instructions about her surgery. The day before her surgery the hospital staff call again to confirm Lita's admission. On arrival for surgery, Lita is immediately booked in, has further assessment and is soon escorted to theatre. Post surgery Lita stays in the EDO overnight. Before discharge the next day Lita is given more information, and pain medication. The hospital rings Lita the next day to confirm arrangements for her post operative check. Six weeks later Lita is able to resume normal activities.
