NSW Rehabilitation Model of Care

The Project

Implementation of the NSW Rehabilitation Model of Care will improve patient outcomes, service delivery, efficiencies and collaboration with health care providers across the health system.

The implementation of the Model of Care will include 4 key deliverables:

  1. Establish governance
  2. Gap analysis and implementation of the 8 good practice principles across 6 care settings
  3. Status of current system activity, utilisation and demand
  4. Realignment of services to meet demands

Principles

The NSW Rehabilitation Model of Care consists of 8 good practice principles.

  1. Leadership
  2. Equitable access
  3. Multidisciplinary care teams
  4. Care coordination
  1. Patient centred care
  2. Evidence based care
  3. Appropriate care setting
  4. Clinical process and outcome indicator

Care Settings

Each good practice principle is applicable across six defined care settings. The new Model of Care will define the intensity of therapy at each setting.

  1. Inreach to acute
  2. Sub-acute inpatient
  3. Ambulatory care - Day hospital
  4. Ambulatory care - Outpatients
  5. Ambulatory care - Home based
  6. Outreach

Patient Journey

There are five key components of the patient journey common to all care settings: 

  • Referrals
  • Access and initial contact
  • Rehabilitation assessment on admission
  • Service delivery
  • Discharge and follow-up

Enablers

Enablers describe the components of the model that are required to support implementation.

  • Workforce
  • Data and performance improvement
  • Care coordination and linkages
  • Technology
  • Appropriate infrastructure
