How do patients flow through the hospital

   
  • Describe the ideal patient journey using the RIC
 
  • Regularly monitor and remap the patient experiences. Include the experiences of carers and/or families.

Policies and Protocols

 
  • Identify policies, protocols and guidelines applicable to the RIC.
  • Clearly identify gaps in existing resources that may limit service implementation.
  • Review and/or develop agreements with GPs, other clinicians, health care and community organisations.
  • Review Literature used to develop RIC Model of Care

  • Establish clear referral processes.
  • Identify the care pathways that will be used.
  • Seek endorsement via the clinical reference groups for the referral pathways.
