The Advance Care Planning model provides a framework for health professionals to improve the journey of people, particularly older people, through the health system. ACP takes into account the patient’s wishes, values and beliefs about medical treatment in order to prepare for end-of-life situations.
In essence, ACP aims to reach a clear and agreed understanding between the individual, their ‘person responsible’ (if appropriate) and treating medical practitioner. A crucial component of successful ACP is that the individual understands that they have choices. As such, discussion is a focus of the model.
