Operating suite & other procedural areas - Handling of accountable items - Standard procedures
Applies to all public hospitals where surgical procedures are carried out to ensure that any items as defined by this document and used during the course of an operation are removed from the patient unless retained intentionally as part of the procedure. All public hospitals are to have regard to this policy in other procedural areas where there is a risk of retained instruments/items (ie labour and delivery units, radiology departments etc). It is also recommended that all licensed private facilities have regard to the document in the development of their own local policies.

NSW Health's Safety Alert Broadcast System (SABS) has been adapted from the UK's National Health Service Safety Alert Broadcast System. Each alert specifies action to be taken by health services, the timeframe in which such action must occur, and specific responsibility for the actions.
