Overview

In June 2008 NSW Health launched a new direction of Chronic Care for Aboriginal People programs and initiatives. This was driven by a number of recommendations resulting from the Walgan Tilly Redesign Project.

All existing and new initiatives designed for Aboriginal people with a chronic disease are now under the strategic program direction of Chronic Care for Aboriginal People (CCAP). The former programs, Aboriginal Vascular Health, Aboriginal Renovascular and Aboriginal Chronic Care Area Health Standards now collectively form the CCAP Program.

NSW Agency for Clinical Innovation

The NSW Agency for Clinical Innovation (ACI) was established in January 2010, as a direct result of the Garling Inquiry into Acute Care Services. The CCAP program was transferred to the ACI in October 2012. The ACI is the primary agency in NSW for promoting innovation, engaging clinicians and designing and implementing new models of care.

Presently the CCAP continues to drive the recommendations of the state wide redesign program as well as developing new initiatives, for example the ‘NSW State Knockout Challenge’ (aimed at improved nutrition and increased physical activity) and ‘One Deadly Step’ (screening, education and follow up). The CCAP program's new direction is focussed on:

  • Treating the person not just the disease
  • Moving away from a “body parts” approach
  • Facilitating local solutions to local problems in addressing access issues to mainstream chronic disease services
  • Supporting the chronic disease work of Aboriginal Medical Services
