This month we explore the current work on severe chronic disease management and catch up on the NSW electronic medical records program.

We are also pleased to share a snapshot of some innovative projects from the clinical leadership program run by the Clinical Excellence Commission.

Connecting Care for Severe Chronic Disease Management Workshop

This workshop reviewed integrated chronic disease management models of care including:

  • Models in Australia and overseas
  • Key issues concerning stakeholder engagement
  • Enhanced use of information and communication technology

Electronic Medical Record (eMR)

The Electronic Medical Record (eMR) is a multi-year program in NSW to replace the paper medical record with an online record which tracks and details a patient's care during the time spent in hospital.

Five web-based training modules have been developed to support eMR:

  • FirstNet (Emergency Department System)
  • Orders
  • PowerChart (Results)
  • Scheduling
  • SurgiNet (Operating Theatre System)

Clinical Leadership Program 2007

The Clinical Excellence Commission's Clinical Leadership Program is multidisciplinary in nature and participants are required to undertake a work-based improvement project, with coaching and support provided by local facilitators.

Some of the projects include:

  • End of Life Care Planning, by Michael Daytner
  • Prevention of Major Joint Infection - Hip, by Debbie Spokes
  • The Mother-Baby Continuum, by Joanne Campbell
  • Falling and Malnourishment - The Occupational Therapy Link, by Rosie Kew
  • Neonatal Transport in Hunter New England, by Paul Craven
