This month, we have patient safety in mind with two new exciting programs aimed at improving clinical handover. Also, check out the NSW Department of Health’s training video, which demonstrates the importance of collecting accurate and complete patient information at registration.

SHARED - A Framework for Clinical Handover
The Mater Health Services (MHS) Clinical Safety and Quality Unit recognised in 2006 that a focus was needed on processes associated with safe patient handovers.
Review of the literature led to agreement on a set of key characteristics expressed in the mnemonic SHARE (S-situation, H-history, A-assessment, R-risks, E-events) which was planned to serve as a “Clinical Handover Guideline” for nursing handovers across MHS.
ISBAR Clinical Handover Project
Hunter New England Area Health Service staff in Clinical Governance are seeking to assess whether a standardised format for communication can improve patient outcome and increase clinician job satisfaction.
The project will test the impact of the ISBAR communication format on the transfer of patients from The Maitland Hospital to John Hunter Hospital.
Collecting Patient Registration Information Training Program
The Collecting Patient Registration Information Training Program was developed by the NSW Department of Health as a tool to enable staff to identify the importance of collecting accurate and complete information, raise awareness of data items that may be difficult to collect and review some strategies that may be used when dealing with difficult situations. This page includes the video produced by the Program.
