Ambulatory Care Coordination Program for Children with Chronic Conditions

The Ambulatory Care Coordination (ACC) Program, launched August 2007, is designed to improve care coordination through prospective integrated health care planning, 24 hour telephone support and strengthening of partnerships between tertiary and community services for those children with chronic conditions who frequently attend ED or remain in hospital for lengthy periods of time.

The section hosts a number of tools and resources on Business Information including the Business Information Competency Model and the Catalogue of Information Assets.

Medical Assessment Unit Workshop

The NSW Health Healthcare for Older People Earlier (HOPE) program is working to deliver better patient journeys for the elderly and those with chronic disease. The establishment of new Medical Assessment Units (MAU) is one of the  key components in the HOPE program.

Over 100 representatives from the sixteen NSW hospital project teams that will implement MAUs met together, on 12 Dec 2007, to discuss progress and their implementation models.

Emergency Department Nursing Forum

ARCHI has a new discussion forum for you to participate in.

The nursing representatives of the newly formed Ministerial Taskforce on Emergency Care in NSW have established a discussion forum with ED nurses, to facilitate the sharing of information and resources.

Can you Help?

ARCHI runs a members-only enquiry service where we access the expertise of our membership base to answer questions.

This month's Questions:

  • Case Load Midwifery
We are looking for information on introducing a case load midwifery model to a small district hospital. We would like to hear from any hospitals that have set up a case load model in their hospital, so that we can learn from their experiences.
  • Echocardiography Examination Waiting Period
What is a "safe" period of waiting time for an echocardiography examination? At what point in time does the risk associated with waiting become unsafe and unacceptable? Does anyone have a Best Practice guideline or Protocol?

If you can help, or have a question to pose to the ARCHI members, please contact the ARCHI office: Phone (02) 4985 5223 or email catherine.knight@archi.net.au

See the Enquiry Service webpage for the enquiries and responses.
