Rehabilitation Redesign Model of Care

Implementation of the NSW Rehabilitation Model of Care will improve patient outcomes, service delivery, efficiencies and collaboration with health care providers across the health system. The Model of Care includes 4 key deliverables:

  1. Establish governance
  2. Gap analysis and implementation of the 8 good practice principles across 6 care settings
  3. Status of current system activity, utilisation and demand
  4. Realignment of services to meet demands

GP/Hospital Clinical Handover Project

The GP/Hospital Clinical Handover Project will build on the existing work to develop elements that are transferable and will aid implementation that is locally appropriate.

It will feature close engagement of clinical and non-clinical representatives from General Practice, hospitals and patient and carer groups to develop a system-wide focus on handover between General Practice and hospitals.

The aim is to improve two-way communication between General Practice and hospitals.

The Project pages on ARCHI have been revised to include the implementation plan, key elements and workshop presentations.

Australian Cancer Trials

Development and evaluation of Australian Cancer Trials: a consumer-friendly cancer clinical trials website

Consumers, led by Cancer Voices NSW, have long sought access to information about cancer clinical trials via a user-friendly website. In response the "Australian Cancer Trials" (ACTO) website was developed and evaluated.

ACTO uses clinical trial registry data (ANZCTR and ClinicalTrials.gov) to allow consumers to search for cancer clinical trials recruiting anywhere in Australia.

Vitamin D information for pregnant women and new mothers

This information brochure about vitamin D for pregnant women and new mothers has been developed and translated into 11 community languages.

The other language versions are available on the ARCHI Innovation Register
