This month we have innovations on a whole spectrum of healthcare. We have beginning and end of life models of care, with the paediatric feeding clinic and the advance care planning models. We also have some new community care tools.

Advance Care Planning Model of Care
The ACP Model of Care has been revised and updated.
The ACP model provides a framework for health professionals to improve the journey of people, particularly older people, through the health system. ACP takes into account the patient’s wishes, values and beliefs about medical treatment in order to prepare for end-of-life situations.

Multidisciplinary Model of Care for Children with Feeding Difficulties
The multidisciplinary Feeding Clinic was established in response to a marked increase in the number of children presenting with feeding problems, either associated with complex medical conditions, or in isolation.
The clinic was developed using an evidence-based integrated approach to complex feeding problems and is staffed by medical, nursing and allied health professionals.

ComPacks Reporting Requirements
A standardised reporting framework has been developed for the NSW ComPacks Program to assist with service management and evaluation.
The web based software reporting tool developed for use by all ComPacks Providers is known as CMaRT (ComPacks Management and Reporting Tool).
Resources to support this tool are now on the ARCHI website.
