Welcome to the new look ARCHI. You may have noticed that the ARCHI website has been redesigned. Our newsletter also has a new name and new look.

Making Change
ARCHI has some new resources designed to help you understand the fundamentals to making change. This will support the implementation of models of care and other change projects.
Making change is in two sections:
- Fundamentals - the case for change, what are you changing, authority and governance, engaging the right people, communications, aligning and leveraging, a global view, setting timeframes, measuring success and tools.
- Designing Change - getting started, diagnosing the issues, designing solutions, implementation that can stick, monitoring and evaluation.

Patient Flow Implementation
We have updated the Patient Flow Systems pages with a new Implementation section with tools to improve patient flow. This section includes:
- self-assessment tool to review your patient flow process

Care Management Plan
The Care Management Plan (CMP) application aims to improve the discharge processes for patients by providing a comprehensive patient – centered discharge tool for multidisciplinary discharge planning.
The CMP is an electronic application developed by the IT department at RPH which is able to be easily accessed from any computer within and across hospital sites.
The CMP templates provide users with standardized care plans that can easily be individualized by users to meet the discharge needs of their patients. This application provides a permanent record of all completed care plans and has the ability to provide data for reports.
