Solutions: Improving the Patient & Carer Experience
We are developing a collection of resources that target the areas for improvement identified by patients and carers. Preview this new section on our site and add your solution.
The solutions cover the 8 Picker Institute dimensions of care:
Access to care; Respect for patient values, preferences and expressed needs; Coordination and integration of care; Information and education; Transition and continuity; Physical comfort; Emotional support and alleviation of fear and anxiety, and; Involvement of family and friends.

Surgical Acute Rapid Assessment (SARA) Model of Care
The SARA Model of Care works to support the Emergency Department in its efforts to reduce access block, improve hospital processes and provide high quality of care.
It focuses on the beginining of the Emergency Department pathway and successfully facilitates patients to prioritised assessment and early discharge and/or referral for review and/or investigation, giving them the shortest stay possible in the Emergency Department.

ARCHI Innovations in Practice Workshop - Pain Management will showcase new innovations in clinical practice, share experiences and lessons learnt and provide practical strategies, tools and tips to improve clinical practice.
The workshop aims to improve quality of care, patient safety and efficiency in Pain Management. Topics will include:

- Chronic Pain Management
- Palliative Pain Management

Chronic Disease Self-Management Support Model of Care
Implementing a Self-Management Model of Care empowers and adds confidence to individuals with chronic disease, their carers and family who actively participate in managing their conditions.
This model of care provides tools and resources for implementing chronic disease self-management support services, as well as describing types of self-management models which may be appropriate.
Integrated Aged & Chronic Care Expo
The Integrated Aged Care and Chronic Care Program encompasses all Clinical Services Redesign Programs for older people and those with chronic disease. This Expo showcases projects from this program.
