Take a look at our new content on this topic: Transcultural Approach to Honouring Mind and Body Program from Queensland, and the MAITRI service from Western Australia. Transcultural Approach to Honouring Mind and Body Program

The Queensland Transcultural Mental Health Centre (QTMHC)’s Transcultural Approach to Honouring the Mind & Body Program aims to tackle the problem of chronic disease and depression amongst people from CaLD backgrounds through exercise, self-management skills and better links with CaLD communities.

Mind & Body includes:

  • depression and chronic disease self-management group sessions where participants work on cognitions, goal setting and problem solving
  • bilingual community health promoters to provide cultural mediation between the culturally diverse communities and the health system, and
  • a focus on making physical activity more accessible to CALD communities.

The program will commence in July 2008 and will run for 4 years. It will be facilitated by bilingual allied health professionals. Initially, it will target adults living with a chronic condition and depression from Indian, Vietnamese, Filipino, and Arabic and Spanish-speaking backgrounds.

MAITRI Mental Health Service

MAITRI is a new independent specialist mental health service for immigrants and refugees. It was established in Western Australia by the Multicultural Service Centre of WA Inc (MSC).

MAITRI seeks to meet the mental health needs of people from culturally and linguistically diverse (CaLD) backgrounds by adopting cultural factors in the management of the condition.

HARK - Health Assessment for Refugee Kids

The vision of the Children’s Hospital at Westmead is ‘Healthy Children for a Healthy Future’ and as part of this vision we have established the HARK service which provides comprehensive health assessment, treatment and support for newly resettled refugee children in NSW. 

The HARK service plays an important public health and preventative role by providing crucial early diagnosis and timely intervention in this very vulnerable group of children.  A key aspect of the HARK service is to enable continuing care in the community.

Tidal Model of Nursing Care

Justice Health and Sydney South West Area Health Service (SSWAHS) are implementing the Tidal Model of Nursing care within two Justice Health clinical units and one SSWAHS unit.

The Tidal Model was developed in the UK and has been applied in nursing work in a variety of contexts internationally. The Tidal Model is:

  • a clear, evidenced based model of care
  • a contemporary nursing model that has been evaluated across a number of similar settings
  • particularly useful in situations where recovery and continuity of care are a central concern

 

Care Management Plan

The Royal Perth Hospital (RPH) has been working on a collaborative program to improve discharge processes for patients who have chronic disease and multi-disciplinary care needs. Care plans are patient centred, evidence based and enhance communication between RPH, the patient’s GP and other care/service providers.

The Care Management Plan (CMP) identifies and records management goals, interventions to meet those goals, the person responsible for those interventions and their contact details.

CMP provides a consumer focused approach to discharge planning with engagement of consumers, carers and health providers. Care plans are written in lay language and provide a comprehensive resource for the transition to community-based care, facilitating patient self-management where possible.
