Developing knowledge and skills for diabetes prevention
The Outback Diabetes & Vascular Health Service (OD&VHS) enhances community capacity building by developing knowledge and skills for diabetes prevention and management in the far western NSW.
This service is a collaborative partnership between Greater Western Area Health Service (GWAHS), Maari Ma Aboriginal Health Corporation and the Royal Flying Doctor Service (RFDS).
The role of the OD&VHS is to identify health professionals in the remote cluster who have an interest in diabetes prevention and management and would like to further their knowledge through an accredited Diabetes Educators Course.
An Integrated approach to treating Diabetes type II and Depression: The "MADE-IT" Program
The MADE-IT Program was developed through a partnership of health professionals and consumers, and addresses both diabetes type II and depression in an integrated manner using evidenced based practice.
The program is delivered in small groups, weekly for eight weeks.
Results on outcome measures show significant improvement in depression symptoms, psychological distress, problem areas in Diabetes, and improved Diabetes knowledge. Physical measures also show improvement.
Rationalisation of Diabetes Services for Aboriginal Peoples in Muli Muli, Casino, Jubullum Village and Coraki Box Ridge
This project developed a multi-disciplinary approach to diabetes management amongst the isolated rural Aboriginal communities of Muli Muli, Casino, Jubullum Village and Coraki Box Ridge.
For the first time, a one-stop shop approach enabled a team of healthcare workers to assess clients at a clinic. The team included Aboriginal Health professionals, a Physician, an Ophthalmologist, a Medical Scientist, Diabetes Educators, a Podiatrist, Registered Nurses and General Practitioners.
Helping Young People Take Control of Their Diabetes and Successfully Transition to Adult Services
The Northern Rivers Area Health Service (NRAHS) Diabetes Centres Project developed a flexible transition program model that could be delivered across the health area, and adapted for each centre's sociodemographic needs.
Ambulatory Stabilisation Program for Newly Diagnosed Children and Adolescents with Diabetes (DDCP)
This program aimed to prevent hospital admissions, reduce length of stay and provide care for children and young people in the best setting for them, which is an ambulatory care model.
The Diabetes Day Care Program (DDCP) is a hospital based model of ambulatory stabilisation in which the child and family travels to CHW Diabetes Centre on a daily basis for the first few days after diagnosis or after early discharge to be provided education and management as outpatients.
