Also included are innovations in improving the health care of children, the care of chronic conditions, continuity of care, community health care, patient education, consumer involvement in health care-related matters, Emergency Department efficiencies, ambulance services, communication between general practitioners and hospitals, admissions, pre-admission clinics, in-hospital activities, management of ambulatory and outpatient services, case conferencing, discharge planning, and approaches to the delivery of health services in rural and remote areas.
