When transferring the care of patients from one health professional to another it is vital that appropriate information is given to ensure patients are receiving best care and treatment.

The solutions below showcase how some health services are working to improve in this area.

Solutions

The Envelope is a big, bright yellow resealable envelope in which to put a resident's medical documents and other key information to 'go with' a resident of an aged care home when the resident is transferred to hospital.

This program aims to increase the uptake of falls prevention strategies for older people through phone follow ups within a week of discharge from the emergency department.

Project to ensure the accuracy of charted medications for patients over 65 years on four or more medications admitted through the Emergency Department.
