Acute Care of the Elderly (ACE) Model of Care
This model is based on Hornsby Ku-ring-gai Health Service's ACE Model of Care. Acute Care of the Elderly (ACE) is a shared care model between physicians, geriatricians and their teams. It integrates comprehensive geriatric assessment into the optimal medical and nursing care of patients in a multidisciplinary environment. The multidisciplinary team focuses on providing the right care at the right time, patient function and patient safety. Consideration is given to the entire patient journey from admission, through the hospital and discharge to appropriate community resources.
This model of care can help meet the special requirements of the health system's most frequent users, the elderly.
