Community Acute/Post Acute Care (CAPAC)
About CAPAC services
Under the CAPAC Model of Care, selected types of acute/post acute care are delivered to suitable, consenting patients at their home as an alternative to inpatient (hospital) care. The essential functions of CAPAC services are to provide the most appropriate care setting, avoid hospital admissions or reduce patient length of stay through the immediate provision of multidisciplinary care.
Hospital based medical staff such as visiting medical officers, staff specialists or other nominated medical officers who have the right to admit patients to the service provide medical supervision to CAPAC patients.
General practitioners are also able to provide medical supervision to the CAPAC teams and in some area health services they are able to refer patients directly to the CAPAC services in the same way as hospital based doctors.
There is a growing body of evidence that demonstrates that CAPAC is as clinically effective as hospital care, delivers as good health outcomes and that there is strong patient and community support for such services.
About APAC/GP Shared Care
In the APAC/GP Shared Care model the GP and the APAC team work closely together to manage the patient in the community. The GPs role is to establish the clinical diagnosis, prescribe and administer the first dose of treatment. The APAC team under the ongoing clinical management of the GP continue the care. This model ensures that where possible the patient does not need to go to hospital for initiation of clinical care. Conditions specifically targeted in this model are those that have not responded to oral antibiotics or other interventions or therapies.
