A Frozen Meal Service – Nutritional Care from Hospital to Home

Abstract

A frozen meal service for external customers commenced operation at SEI Food Service in January, 2006. This proactive and cost-effective initiative represented a rung in the “continuum of care” ladder, allowing patients to continue to have nutritious, safe, tasty and economical meals at home after discharge. The service was later extended to the general community, widening the customer base. Meals could be accessed through the food service department, the Meals-on-Wheels (MOW) coordinator, and frozen meal banks at each hospital site.

The service fits within a primary health model to reduce community malnutrition and hospital readmissions. Further, it may represent an opportunity for earlier patient discharge, especially where access to prepared, nourishing meals in the home is limited.
