The NSW Health HOPE Program is working to deliver better patient journeys for the elderly and those with chronic disease. They are our largest and fastest growing patient groups, and their medical treatment is often complex and ongoing. The challenge is to keep the elderly and people with chronic disease safely supported in their familiar home and community environment, this is what we are aiming for with the HOPE Program. |
