Cardiology Models of Care

There are three components to the Cardiology Model of Care, each designed to address different patient requirements.  Click on a section of the Ideal Journey Map below to find out more about implementing each model.

 

The Cardiology Model of Care described here has been developed as part of NSW Health's Clinical Services Redesign Program (CSRP). CSRP seeks to fundamentally redesign health care systems to ensure that each patient journey is safe and of the highest quality.

These pages outline the models of care required to support the ideal cardiology patient journey. They provide examples of local, national and international best practice care.  They present a distilled set of possible solutions for cardiac care, supplemented with experiences from other jurisdictions (national and international) and from the published literature.
