sStrengthen Primary Health and Continuing Care in the Community: 2008 NSW Health Awards

Primary health services include general practice, community health centres and nursing services, youth health services, pharmacies, allied health services, Aboriginal health and multicultural services.  They are provided in both public and private settings.  For most people they are the first point of contact with the health system. They are also the services people tend to use most.

What we are striving for in 2010 is a health system that helps people to access most of the healthcare they need through an integrated network of primary and community health services across the public and private health systems. These community-based services will be linked to and backed up by hospital services as needed, enabling people to access Commonwealth and State health programs.  Early intervention principles will be embedded into NSW Health‘s service delivery. Early intervention will lead to improved health outcomes and reduced avoidable hospital admissions.

  • Alternatives to Acute Hospital - APAC/GP Shared Care - Northern Sydney Central Coast Area Health Service
    The Northern Beaches (NBs) APAC/GP Shared Care model was initiated in 2006/2007 in response to increasing demand on Emergency Departments (EDs) and the growing number of elderly patients, with ambulatory sensitive conditions, being admitted to hospital.

  • Wound Management Model Redesign - Hunter New England Area Health Service
    Project to implement an evidence based wound management model across community health sites in Greater Newcastle Cluster which identifies high-risk clients and further reduces the number of clients suffering from chronic wounds.

  • CKD Services to Aboriginal People - North Coast Area Health Service
    Project to provide culturally appropriate CKD services to the Aboriginal & Torres Strait Islander population in the Richmond Network in collaboration with primary and community based services to prevent disease progression.

  • Driving Change in Mental Health - South West Area Health Service
    The clinical redesign of community mental health in one of the sectors of SWAHS was identified as an area for improvement due to issues in service capacity. This was impacting on length of stay (LOS) of the inpatient unit as well as the number of clients seen by the service.

  • Dry as a Bone Solutions - Greater Western Area Health Service
    The project identified and addressed the continence needs of our elderly and disabled clients in the community of Broken Hill.


  • Improving Access to Sexual Health Services - Greater Southern Area Health Service
    The project aimed increase Sexual Health, HIV and Blood Borne Virus services to Aboriginal clientsattending Munjuwa Aboriginal Corporation in Queanbeyan, NSW.

  • Kings Cross Youth at Risk - South East Sydney Illawarra Area Health Service
    The Kings Cross Youth at Risk Pilot Project was established in 2005 at Kirketon Road Centre as a coordinated interagency early intervention strategy for young people.

  • Managing the Patient, not the Admission - South East Sydney Illawarra Area Health Service
    The Community & Outpatient Nutrition Extended Care Team (CONECT) was established following critical evaluation of current acute hospital based dietetic practice and identification of factors contributing to poor patient compliance with nutritional therapy post discharge.

  • Not Forgotten - Childrens Hospital Westmead
    Sibling Camp aims to assist siblings to form ongoing relationships with other siblings, enjoy recreational activities without the burden of caring and explore sibling issues.

  • Nurse Triage at Sexual Health Centre - South East Sydney Illawarra Area Health Service
    Publicly funded Sexual Health services will prioritise access to and focus of services to those priority groups most in need as delineated by NSW Health.

  • Physical Health for People with Mental Illness - North Coast Area Health Service
    The project aim of providing the physical assessments to at least 60% of the CMH clients was achieved within the initial 12 month timeframe. Mechanisms have been identified to increase partnerships between the CMH team, General Practitioners (GPs), and their clients in relation to physical health needs.

  • Speech Pathology in Schools (SPinS) - Greater Southern Area Health Service
    A speech and language assessment tool was used to screen all kindergarten children across targeted primary schools within the Wagga area. The results were used to tailor an intervention program.
