Abstract

Fairfield Ambulatory Care has developed "The Fairfield FIT" (Falls Intervention Team); a multidisciplinary, multifaceted falls prevention program. At risk clients are referred from the community and through screening non-admitted Fairfield Emergency Department (ED) patients with a history of fall. Patient intervention is planned and coordinated. In the 6 month period between October 2006 and March 2007, more than 100 clients were referred to the FIT program and received a variety of interventions. To date the results are very encouraging with a core group showing a 64% reduction in their falls.
