Abstract
A prospective study of pharmacy-initiated interventions at the Sydney Children’s Hospital (SCH) oncology unit was undertaken to determine the volume and scope of medication errors and demonstrate the impact pharmacists have in improving the quality and safety of patient care. The NSW Information Management System (IIMS) was utilised to report all pharmacist interventions from July 2006 to June 2007. A multidisciplinary panel analysed a total of 391 interventions for 325 patients. The data was also examined in relation to junior medical staff rotations to identify trends. A series of targeted actions have been implemented and outcomes have been evaluated. Pharmacist interventions in childhood cancer therapy have demonstrated measurable improvements in safety.
