Abstract
The impetus for the introduction of DANGERS at Shellharbour Hospital followed the review of documentation relating to patients who had experienced a significant deterioration in their condition or an adverse outcome.
On case review, it became evident that patient observations were not being taken as regularly as one might expect given the overall condition of the patient, or that despite unfavourable observations having been recorded, appropriate nursing and/ or medical intervention did not always necessarily occur in an acceptable time frame.
With the cooperation of senior medical and nursing staff, a medical emergency team was developed using existing staffing resources.
Ongoing support measures have been put into place to ensure the success of DANGERS at Shellharbour Hospital.
